POSITION SUMMARY:

Reporting to the Chief Medical Officer (CMO), the full-time, exempt Physician, Family Practice Primary Care Provider (PCP) is a foundational member of a high-performing integrated healthcare team. The PCP is responsible for providing high-quality primary medical care in partnership and collaboration with Honor Community Health’s (HCH) integrated practice team, in support of HCH’s mission of delivering comprehensive integrated care to vulnerable populations of Oakland County. The PCP is responsible to the CMO for clinical quality and outcomes, as well as productivity.  The PCP will adhere to HCH policies and procedures in accordance with the policies established by Honor Community Health. The PCP will work collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality health care to all.

Essential Functions:

Clinical Interventions

  1. Obtain a complete history, perform a physical examination, and provide a professional diagnosis and treatment plan as part of the evaluation and management of a patient presenting with a specific medical problem(s) or concern(s). 
  2. Evaluate and treat acute and chronic illness, including developing a treatment plan and assisting with coordination of care.
  3. Perform minor surgical procedures.
  4. Document patient care encounters (i.e. history and physicals, patient and patient-related communication both during and after hours, etc.) in the patient’s electronic medical records accurately and within 72 hours of the patient encounter.  
  5. Make appropriate referrals to both internal and external specialties within the organization and/or to non-organizational specialties when such referrals are medically necessary.  Provide consultation to other providers, Behavioral Health Consultants (BHCs), and Resident Physicians on as needed basis.
  6. Work collaboratively with members of the health care team, including but not limited to Podiatrists, Dentists, Nurse Practitioners (NPs), Behavioral Health Consultants (BHCs), Physician Assistants (PAs), Medical Office Assistants (MOAs), Resident Physicians, Medical Students and physicians from other specialties.
  7. Prescribe medication(s) in accordance with Michigan statute and professional practice guidelines.
  8. Complete all charts and review and sign off on all incoming reports (e.g. lab, imaging) and follow up within 72 hours of the patient encounter.
  9. Partners with HCH behavioral health and dental professionals and staff, embrace and apply the HCH integrated primary care model.
  10. Provide after-hours telephone coverage, as assigned by CMO. 
  11. Adhere to Honor Community Health’s established productivity standards.
  12. PCP will be a preceptor and/or mentor to Medical students as needed.
  13. In partnership with HCH behavioral health clinicians, dental professionals and staff, embrace and apply the HCH integrated primary care model.

Quality Assurance/CQI Improvement

  1. Demonstrate ability to learn and implement quality improvement practices by engaging in literature review professional resources for the purpose of self-evaluation, lifelong learning, and practice improvement.
  2. Identify, analyze, and adopt new knowledge guidelines, standards, technology products, or services that have been demonstrated to improve outcomes.
  3. Assist the Associate Chief Medical Officer and CMO in establishing medical policies, and evaluating current practices, policies, and procedures.
  4. Participate in meetings such as monthly clinician/provider, all-staff, peer review, quality improvement and assurance meetings, grand rounds, family conferences, huddles, etc. Participate on the Honor Community Health Quality Committee; assist in the development of quality improvement policies and procedures.
  5. Recommend changes in all clinical areas, and/or in general areas to improve patient flow, medical records, billing practices, and appointment practices.  Responsible for direction and supervision of clinical staff, as assigned by Associate Chief Medical Officer and/or CMO. 
  6. Participate in quality improvement initiatives and activities as directed by the Associate Chief Medical Officer and/or the Chief Medical Officer. 
  7. Exhibit self-awareness to identify strengths, address deficiencies, and recognize limits in knowledge and expertise.
  8. Participate in the implementation of and follow Meaningful Use and Patient Centered Medical Home guidelines.  Participate in peer review process, providing expert and unbiased review of peer physicians and/or mid-level providers.
  9. Understand the use and allocation of resources to ensure the practice of cost-effective care while maintaining quality of care. 
  10. The expected benchmarks are:
  1. PCP will provide 4,000 visits per year
  2. PCP will be on the on-call schedule for all HCH sites/patients.
  3. PCP will be a preceptor/mentor for students if applicable.
  4. PCP will take the lead on a special project or committee.

Community Outreach/Collaborations

  1. Works effectively with other health care professionals and community partners including but not limited to Community Mental Health, core provider agencies, local health departments, local hospital networks, etc. to provide collaborative, patient-centered care while maintaining a climate of mutual respect, dignity, diversity, ethical integrity and trust.
  2. PCP will provide health education if needed or requested to health center staff. 
  3. PCP will participate in community outreach activities as requested by the Associate Chief Medical Officer or CMO.
  4. Performs other duties as assigned.  During a public health emergency, the employee may be required to perform duties similar to but not limited to those in the job description. During clinic operation shortages and without advanced notice, the PCP may be relocated to a different HCH location.

What are we looking for?

  • Graduate of accredited medical school and satisfactorily completed an ACGME-accredited training program in Family Medicine or Internal Medicine.
  • Must be board certified at hire and will be expected to maintain appropriate American Board of Medical Specialty (ABMS) board certification, licensure, and membership in professional organizations.
  • Must have unrestricted license to practice medicine in the State of Michigan.
  • Eligible to obtain and maintain a federal (DEA) and state narcotic registration.
  • Eligible for hospital privileges. 
  • Eligible to obtain and maintain credentials as a Medicaid and Medicare provider.
  • Must have current BLS and ACLS certification.
  • 1-2 years of progressive experience as a clinician.
  • Experience with CPT and ICD coding.
  • Strong computer skills with experience using Electronic Dental Records (NextGen preferred) and Microsoft Office.
