POSITION/ TITLE: Health Homes Program Manager
SUPERVISED BY: Director of Community-based Services
JOB DESCRIPTION:
Under the general supervision of the Director of Community-based Services (DCS), the Health Homes Program Manager (HHPM) will work in a fast-paced Human Service environment and have the ultimate responsibility of HONOR’s Health Homes (HH) programs. Health Home Care Management is a service model where all the professionals involved in a person’s care communicate with one another so that all their needs (medical, behavioral health and social service) are addressed in a comprehensive manner. The HHCM will manage the day-to-day activities of HH. Regular interaction with Agency Management is expected in order to provide program updates and report on the state of the program. HHPM will work with coworkers, supervisors and staff from other departments to solve problems and to improve processes and services. The HHPM must ensure that the outreach strategies and referrals to HH are fulfilled and maintained and that expected participation rates and billing are consistently met. The HHPM must have full working knowledge of all NYS Department of Health and Hudson Valley Care regulations, policies and procedures. This position is responsible for being on-call 24/7 to provide information and emergency consultation services for client care.
QUALIFICATIONS FOR THE POSITION:

1. Master’s degree in a human service discipline or Licensed level healthcare professional (RN, Licensed Clinician, etc.)
2. Two years’ experience in Human Service Field (preferred in providing services to medically underserved populations) with at least two year’s managerial experience in a behavioral health setting.
3. Knowledge of NYS Health Homes preferred.
4. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people.
5. Strength in hiring, recruiting, managing, developing, and coaching a strong team of staff.
6. Detail-oriented and have excellent organizational, communication and interpersonal skills (friendly, courteous, helpful, ability to work as part of a team).
7. Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR’s mission.
8. Ability to effectively present information and respond to questions from groups of clients, other Agencies and the general public.
9. Ability to function independently and have flexibility, personal integrity and the ability to work with residents, staff and support agencies.
10. Ability to use MS Word, Excel, Electronic Health Record (database system)
11. Must have and maintain a valid driver’s license.
Job Type: Full-time
