About This Homecare Clinical Manager Registered Nurse (RN) Position
HopeHealth is seeking a Registered Nurse (RN) Clinical Manager for its affiliate HopeHealth Visiting Nurse. In this role you will lead our clinical field staff in delivering exceptional patient centered care in the home setting.
This is a full-time position that is eligible for benefits. Position also rotates covering holidays, manager-on call, and weekends when the weekend supervisor is off.
This position will be based in our Lincoln, RI location.
What will a Homecare Clinical Manager Registered Nurse (RN) do?
- Direct and coordinate patient care
- Review clinical documentation, including OASIS, and patient's plan of care
- Monitor clinical outcomes, and overall utilize case management strategies to work with Case Managers to manage caseloads and oversee visit utilization
- Participate in ongoing education, evaluation, and development of staff
- Conduct field visits to supervise staff with regards to infection control, clinical assessment skills and point of service documentation
What are the qualifications for a Homecare Clinical Manager Registered Nurse (RN)?
- Registered Nurse with bachelor’s degree in nursing
- Minimum five (5) years nursing experience with in-depth clinical knowledge to include experience working for a Medicare Certified Home Health Agency. Prior supervisory experience a plus.
- Comprehensive knowledge of OASIS and Medicare Conditions of Participation for home health.
- Prior experience with electronic medical documentation and reporting systems.
- Strong clinical assessment and case management skills.
- Strong active listening skills and communication skills in both verbal and written communication.
- Strong critical thinking skills and the ability to juggle multiple assignments at the same time.
- Currently licensed in Rhode Island and Massachusetts
