We are seeking a dedicated Registered Nurse (RN) to join our Crisis Residential and Rehabilitation program in Norristown, PA. Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse with over 100 locations throughout Southeastern Pennsylvania and the state of Delaware.
Responsibilities and Duties
The Registered Nurse is responsible for conducting comprehensive physical health assessments, including mental status, use of drugs or alcohol; activities of daily living (e.g., self-care) within 24 hours of admission and throughout stay, as needed. Provide on-call crisis intervention covering nighttime and weekend hours for health and medication concerns. Coordinate Medical care for participants who have medically complex needs or for participants who have acute problems, which become medically complicated. Coordinate information flow and communication between Specialist and Primary Care physicians and between professionals and program support staff. Facilitates questions regarding medical diagnosis, or treatment plans; interface with physician and relay appropriate information to support staff.
Qualifications and Skills
- • Board Certified and Licensed Registered Nurse in the State of PA.
- • Prior experience in servicing community mental health facilities and participant needs population.
- • Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices.
- • Ability to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preferences in treatment are essential.
- • Excellent oral and written communication skills.
- • Strong interpersonal skills.
- • Very strong organizational and analytical skills and detail oriented.
- • Must be available for scheduled emergency on-call service.
- • Valid driver’s license and acceptable driving record.
