Hospice of Chattanooga is currently hiring full-time Compassionate On-Call Registered Nurses (RN) to provide patient care in and around the Chattanooga, TN Area on weekends!
*** $5,000 Sign-On Bonus ***
The On-Call RN is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the hospice patient and family. The On-Call RN utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. The On-Call RN assumes on-call coverage for the designated teams and functions as an Interdisciplinary (IDT) team member to provide hospice care, support , and interventions after hours.

Responsibilities
- Provides emergency, PRN, routine, and admission visits as necessary or scheduled.
- Assumes primary responsibility for hospice patients during on-call hours.
- Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions.
- Reviews clinical referral information and responds to requests and inquiries as appropriate Collaborates with Administrator On-Call regarding the eligibility and appropriateness of client for hospice services.
- Assesses and evaluates patient's status by: following the plan of care, regularly reevaluating patient and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing interventions/actions. Includes the patient and the family in the planning process.
Qualifications
- Bachelor's degree in nursing or health care preferred.
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Hospice/home health experience preferred but not required.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification.
- CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.
