What We Are Looking For

Job Title: Full Time Triage RN

Schedule: Flexible Evening/Overnight Shift Hours (Work with us to determine a schedule that works best for you)

A vital component of this position is communication and collaboration with all members of the care team to ensure a seamless transition/hand off between shifts for the purpose of continuity of care. This position will be on-call to ensure we provide the best care possible 24/7 to each of our patients as we experience an increasing patient census and growing need for hospice services within the community.

What We Need

• Graduate of an accredited school of nursing. One to two years of recent medical surgical experience in an institutional setting, home care or skilled nursing facility.
• Current RN licensure in NY State and driver’s license
• Present a positive image of Hospice in all contacts.
• Provides professional nursing care by utilizing all elements of nursing process
• Communicates with Hospice IDG members on a timely basis regarding changes in patient’s condition that occurs during their shift
• Provide timely visits to home, hospital or skilled nursing facility in any part of Orange/Sullivan counties anytime during assigned shift.
