What is a Community Field Nurse?
The role of a Community Field Nurse is to assist Case Management with admitting patients onto the hospice program, routine visits, and as needed visits. The Community Field Nurse will complete comprehensive assessments of patients, identify their care needs, and determine appropriate goals and interventions to meet those needs. Since this position requires a commitment of every Saturday or every Sunday, we provide an enhanced pay rate for those in this role.
