Responsible for mentoring and educating clinical team members within the agency.

WHAT YOU WILL DO:

• Provides clinical guidance according to policies and procedures, addresses questions and concerns, and serves as a resource to clinical team members.
• Assists with orientation and mentoring clinical team members.
• Reinforce agency practices and policies with clinical team members.
• Assist with updating, implementing, and maintaining Preceptor Program
• Aligns clinical guidance with strategic initiatives.
• Assists in identifying learning needs of staff through formal and informal needs assessment.
• Perform observatory supervised visits for team members in coordination with their performance evaluations.
• Serves as a consultant to staff caring for patients.
• Creates transition plans for team members transitioning to another team.
• Assists with the understanding and use of the EMR.
• Assists with the education and development of corrective action process for staff.
• Serves on committees as assigned.
• Performs other duties as assigned

SUCCESS CAPABILITIES

• Licensed RN in State of Ohio
• MSN or CNE Preferred
• Experience in hospice care, preferred experience with Hospice of the Western Reserve.
• Supervisor experience preferred

CONDITIONS OF EMPLOYMENT:

• Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
• Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
• Provide an active driver’s license.
• Ability to provide proof of eligibility to work in the United States.
• High level of integrity, ethics and professionalism.
