Responsible for mentoring and educating clinical team members within the agency.
WHAT YOU WILL DO:
- Provides clinical guidance according to policies and procedures, addresses questions and concerns, and serves as a resource to clinical team members.
- Assists with orientation and mentoring clinical team members.
- Reinforce agency practices and policies with clinical team members.
- Assist with updating, implementing, and maintaining Preceptor Program
- Aligns clinical guidance with strategic initiatives.
- Assists in identifying learning needs of staff through formal and informal needs assessment.
- Perform observatory supervised visits for team members in coordination with their performance evaluations.
- Serves as a consultant to staff caring for patients.
- Creates transition plans for team members transitioning to another team.
- Assists with the understanding and use of the EMR.
- Assists with the education and development of corrective action process for staff.
- Serves on committees as assigned.
- Performs other duties as assigned
SUCCESS CAPABILITIES
- Licensed RN in State of Ohio
- MSN or CNE Preferred
- Experience in hospice care, preferred experience with Hospice of the Western Reserve.
- Supervisor experience preferred

CONDITIONS OF EMPLOYMENT:
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
- Provide an active driver’s license.
- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics and professionalism.
