Team Description: After Hours staff provide triage and emergency support and management of hospice patients in their individual home or long term care environments throughout our service area. Staff works scheduled shifts to bridge care and support to our patients and caregivers between their assigned interdisciplinary teams and after hour needs.
Position Summary: Responsible for responding to telephone requests for assistance and perform home visits for our patients and families during the evening or on weekends. The primary function is to assess and identify patient/family needs, establish nursing patient/caregiver goals and identify and implement interventions.
This role provides much needed, compassionate care for our patients, meeting them where they are, while also allowing for the autonomy necessary to maintain the work/life balance we all strive for.

Qualifications
A Diploma or Associates Degree in nursing is required; Bachelor’s Degree preferred.
A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
Current CPR required; CHPN preferred.
Minimum 2 years related experience; Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
Experience in direct care of patients with life limiting illnesses required.
Computer proficiency is required.
