Position Summary
The RN Case Manager coordinates and administers hospice nursing care for patients, families, and caregivers as part of the Hospice Team. The RN Case Manager is well experienced in nursing with an emphasis on providing care to high acuity/highly complex patients. The RN Case Manager utilizes various members of the Hospice Team and community resources to coordinate and provide services outlined in a plan of care focused on the needs of the patient, family, and caregiving unit.
Experience
Minimum of two years experience – preferably in the ICU/ER
Experience in Hospice Care preferred
Experience in Acute Care setting preferred
Understand hospice philosophy and issues of death/dying
Education and Training
Associate degree in Nursing is required
Bachelor’s Degree in Nursing is preferred
Licenses, Certification and Registration
Possess and maintain current CPR certification
Current Massachusetts licensure – Registered Nurse
Valid Driver’s License, reliable automobile and required insurance
Hospice and Palliative Care Certification beneficial
Job Type: Full-time
Pay: $30.00 - $40.00 per hour
