AGENCY DESCRIPTION: 

Huckleberry Youth Programs (HYP) is a critical provider of essential safety net services for high-needs youth and their families in San Francisco and Marin County. For over 50 years, HYP has developed and maintained prevention programs that promote the health and leadership of youth, and, as a result divert youth from violence, crime and gang involvement, as well as other harmful behaviors, including running away, drug/alcohol abuse, risky sexual activities, and involvement in the juvenile justice systems. POSITION SUMMARY: 

Huckleberry Youth Programs Project READY (PR) supports youth at Martin Luther King Jr. and Willie Brown Middle Schools. PR is a middle school transition program that provides case management and family engagement focused on supporting youth with their academic and social-emotional skill development. Youth participants are referred from our partner schools when they are struggling with their academic and/or behavioral performance. The ultimate goal is to support them during their 8th grade year and have a successful transition into high school. The Case Manager is a bridge between the program, the youth, their family, their community and the school partner sites. The Case Manager must be able to work within a collaborative process, include families in services, and negotiate successfully with multidisciplinary systems such as middle and high schools.  

ESSENTIAL FUNCTIONS: 

  1. Facilitate the recruitment and outreach of prospective youth and family participants.

  2. Conduct face to face and/or virtual assessments of youth to identify short/long term goals. 

  3. Work collaboratively within the Project READY team to develop interventions and supports for the youth and their families.  

  4. Develop and facilitate care planning with specific goals related to their academic and social-emotional learning and development (SEL/D).  

  5. Network with other youth providers to establish liaisons and resources.  

  6. Maintain ongoing caseload and consistent electronic documentation of contacts with youth, their families and community partners. 

  7. Engage participants’ families in the program regularly.  

  8. Make and maintain contact with school officials, counselors and others associated with the youth.  

  9. Act as liaison to the school site and administration.  

  10. Co-facilitate, plan & coordinate a 16 week group for program participants 

  11. Participate in ongoing training. 

  12. Coordinate and attend outings for participants.   

  13. Conduct home, community and school visits on a weekly basis for participants. 

  14. Assist in the development and implementation of summer intensive programming for rising 8th and 9th graders. 

  15. Participate in site staff, team and agency meetings/trainings.  

  16. Other duties as assigned.

  9. Exercise discretion and professional judgment at all times in keeping with the responsibility carried personally and by the agency for the care and welfare of youth and families. 

WORK ENVIRONMENT: 

This position primarily works out of the partner school sites. Staff will continue some hybrid work including remote & in-person service delivery. Our office is located at 44 Gough St. This role routinely includes driving and transporting youth. The position also includes the use of standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. The office is located in Hayes Valley/Civic Center neighborhoods of San Francisco and is accessible by public transportation but also has a parking lot for staff.     

PHYSICAL DEMANDS: 

This role requires the ability to talk, hear and sit, to drive your personal and agency vehicles, to be able to lift and/or move up to 10 pounds unassisted.  

Qualifications


REQUIRED EDUCATION AND EXPERIENCE: 

1.  A combination of three years of college and/or relevant experience working with youth and their families. 

PREFERRED EDUCATION AND EXPERIENCE: 

  1. Knowledge of community-based agencies serving adolescents in SF Bay Area strongly preferred 

  2. Past experience successfully working with marginalized communities strongly preferred. 

  3. Experience working with the SFUSD

  4. Bilingual Spanish - able to perform all areas of job responsibilities in Spanish.

ADDITIONAL ELIGIBILITY QUALIFICATIONS: 

  1. TB clearance 

  2. Valid CA driver’s license & clear DMV record 

  3. 2 years driving experience 

  4. Reliable transportation

  5. Bilingual in Spanish & English preferred, stipend provided
