What are the responsibilities and job description for the LEAD Case Manager II position at Human Resource Development Associates, Inc.?
Human Resource Development Associates, Inc. is accepting applications for the following position: LEAD Case Manager II: Providing case management services to participants referred to the LEAD Program. Responsibilities will include: 1) Engagement and Outreach 2) Intake, Assessment and development of an individualized Care Plan 3) Implementing that Individualized Care Plan 4) Care coordination 5) Program Outreach and Agency Representation 5) Staff Development and 6) Record keeping all under the direction and supervision of the Executive Director and Clinical Supervisor of the Agency.
Minimum Requirements: Bachelor of Arts Degree in Social Work or a related field from an accredited University, at least two years of prior case management experience dealing with clients having a history of mental health, substance abuse and/or populations of person in need. Salary dependent on experience and licensure.
