LPN/RN Care Mgr

Job Overview
The Resident Care Manager is responsible for coordinating with team leaders and care managers to provide comprehensive care to the residents. RCM will oversee the admission and discharge process providing excellent customer service to residents, family, and general public.

Essential Duties

• Supervise nursing activities in the facility during your shift.
• Assist in developing and maintaining nursing objectives and standards.
• RN or LN designee will visit/assess patients exhibiting signs/symptoms of condition change.
• Ensure that each resident’s right to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights is followed by all nursing personnel.
• Assure that residents have a clean, safe, orderly, and comfortable environment.
• Assist in the training of department personnel.
• Meet with residents or family members as necessary.
• Perform administrative requirements such as completing necessary forms, reports, etc., and submitting to DNS as required.
• Completes resident assessment for admit, discharge, and change of condition. Assist in evaluating and assessing medical, social, psychological, spiritual, restorative, and rehabilitative care plans, as well as instructions in self-help.
• Maintain confidentiality of all pertinent resident care information to assure that residents’ rights are protected.
• Assure that a stock level of medications, medical supplies, equipment, etc., is always maintained at adequate levels.
• Consult with the residents’ physician in planning resident care, treatment, rehabilitation, etc., and periodically review and update resident care plans as necessary.
• Assist in developing nursing care plans for individual residents.
• Notify the residents attending physician, as well as the resident’s legal guardian/ representative, when the resident becomes critically ill, injured, etc., as outlined within our established policies.
• Evaluate emergencies and determine emergency measures to be taken.
• Make independent decisions regarding nursing care.
• Notify physician of accidents and complete incident report forms.
• Assure that the nursing needs of residents are met.
• Assist in establishing the administration, procedures, and control of medication.
• Provide and update documentation for physician rounds. Make rounds with the attending physician as necessary.
• Assist the nursing personnel in performing nursing procedures as necessary.
• Additional duties as deemed necessary and appropriate by the DNS.

Other Duties

• Performs tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
• Meet on-going requirements to maintain current Nursing license.

Basic Qualification
Education/Training:

• A nursing degree from an accredited college or university and a current nursing license for the State of Washington is required. Minimum Data Set (MDS) experience is required. A current CPR card is required.

Skills:

• Moderate reading, writing, grammar, and mathematics skills; proficient in interpersonal relations and communicative skills; auditory and visual skills, ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less.

Experience:

• A minimum of 3 years nursing experience in a long-term care facility and 1 year management experience is required.
