RN Case Manager

Employment Type: Full time Shift: Day Shift Description: RN Case Manager - Monday thru Friday with rotating weekends and holidays The Care Manager (CM) is accountable for proactive coordination and timely transition of assigned patients to the most appropriate level of care along the continuum. This position will impact key results such as achieving top decile performance in length of stay, cost efficient resource utilization, preventing readmissions and unnecessary emergency room visits. The CM will work collaboratively with Physicians, Nursing, members of the multidisciplinary team, Population Health, Home Care and PCP offices as well as other resources external to the organization CARE COORDINATION: Responsible for patient assessment of complex clinical discharge needs and establishment of an appropriate plan of care with multidisciplinary team. 1. Demonstrates critical thinking skills and oversees the care coordination of defined population. 2. Ensures that patient goals are set with patient/family, negotiates healthcare resources and educates patient/family regarding available options of care throughout the continuum. 3. Demonstrates clinical competence to care for a specific patient population. 4. Conducts daily multidisciplinary rounds demonstrating ability to identify and address care coordination barriers. 5. Facilitate patient education for safe and effective discharge.
