Job Description:
- The Registered Nurse prescribes, coordinates, and delegates nursing care.
- Responsible for providing direct care, planning, monitoring and evaluating the effectiveness of interventions and maintaining a knowledge base regarding the current nursing practice to include the legal aspects of nursing care.
- Assigns nursing care activities on a shift basis and is responsible for promoting and enhancing the nursing practice on the unit.
- Demonstrates ability to assess patients in a holistic manner and follow the nursing assessment policy. Completes the mental status exam summary of the admission assessment to include all factors and needs of the patients.
- Initiates a comprehensive, thoughtful treatment plan related to patient needs.
- Initiates the Plan of Care within four hours of admission, prioritizing needs.
- Utilizes CPC nursing standards of practice appropriate to assessment, planning, implementation and evaluation of patient care.
- Demonstrates skill and ability needed to provide quality patient care according to nursing standards of practice.
- Identifies and communicates abnormal diagnostic data to the physician.
- Always displays a professional attitude and acts as a role model for nursing staff.
- Treats patients with dignity and upholds patient rights at all times.
- Makes regular rounds on the unit to observe for pt changes and safety.
- Observes, evaluates and reports changes in pt medical/psychiatric condition
- Demonstrates ability to evaluate pt care outcomes and revise plan of treatment as necessary.
- Reviews treatment plan with patient and assists in meeting goals.
- Demonstrates ability to accurately take both verbal and written orders, transcribe, and safely administer medications.
- Accurately documents o the MAR, follow all medication policies, including FDR and monitoring for ingestion.
- Evaluates and reports positive and negative responses to medications.
- Keeps aware of medication side effects.
- Completes medication teaching with patients and families.
- Demonstrates ability to provide pt and family education related to illness, medication, and treatment plan. Able to facilitate psycho-educational groups.
- Demonstrates skill at processing and documenting therapeutic assignments.
- Documentation in patient chart is timely, relevant and specifically related to individual treatment plan and patient goals.
- Handwriting and signature are legible.
- Displays knowledge of and follows ‘AIR’ documentation guidelines.
- Monitors patient whereabouts.
- Maintains 15 minute checks and 1:1 observations as required.
- Coordinates staff assignments taking into account staff skill level, pt and unit needs.
- Completes all work in a timely fashion and easily makes adjustment in duties when milieu requires it. Effectively handles crisis situations in a non-challenging manner using communications and mannerisms intended to de-escalate the situation.
- Communicates with Coordinators or DON ongoing information regarding patient and milieu status. Communicates pertinent info to physician regarding pt status, orders and treatment changes.
- Utilizes Seclusion / Restraint only after all other interventions have failed.
- Follows S/R policies.
- Demonstrates the knowledge and skill necessary to provide care based on physical, developmental, psychological, educational, and safety-related criteria based on age/population served.
- Demonstrates knowledge and ability to implement behavioral management, individual protocols and implementation of same and established program philosophy.
- Demonstrates the ability to accurately take vital signs and reports abnormal values to physician. Demonstrates ability to use and maintain Glucometer and report abnormal values to physician.
- Follows Standard Precautions, Infection Control and Safety policies.
Requirements:
- 2 years psych experience
- BLS
