The Ambulatory Transitional Care Coach is responsible for providing outreach and care coordination to patients recently discharged from an inpatient or observation hospital setting. The goal of this position is to aid in the scheduling of transition of care visits within 7- 14 days post-discharge. This position also provides patient support for referrals and community resources that encourage quality outcomes, smooth transitions, and coordination of care across the care continuum. This position is accountable for working with and representing Independence Health System across multiple internal and external partners.
