Job Title: LPN/LVN (Registered Nurse - Skilled Nursing Facility)
Contract: 13 weeks
Location: Montour Falls, New York
Shift: 3:00 PM - 11:00 PM, 8-hour shift, Evening (40 hours per week)
Job Description:
- Schuyler Hospital is seeking a dedicated and compassionate LPN/LVN to join our team at our Skilled Nursing Facility.
- As an LPN/LVN in our facility, you will play a crucial role in providing high-quality care to our residents.
- This position requires a valid LPN license in the state of New York, as well as current BLS and ACLS certifications issued by the American Heart Association.
Responsibilities:
- Provide nursing care to residents under the direction of registered nurses or physicians.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents.
- Administer medications and treatments according to physician orders and facility policies.
- Monitor resident's condition and report any changes or concerns to the appropriate healthcare professionals.
- Assist residents with activities of daily living, including bathing, dressing, eating, and mobility.
- Document all nursing interventions, resident responses, and other pertinent information accurately and timely.
- Participate in staff meetings, in-service training, and continuing education opportunities to enhance professional growth and development.
- Maintain a safe and therapeutic environment for residents and staff.
Qualifications:
- Valid LPN license in the state of New York.
- Current BLS and ACLS certifications issued by the American Heart Association.
- Previous experience in a skilled nursing facility or long-term care setting preferred.
