Innovare Health Advocates has a unique open position for a licensed home health RN to work with our clinical team at our Internal Medicine offices.

Innovare is a forward-thinking Internal Medicine office with shared team responsibilities with clinicians, other clinical and administrative staff and daily patient contact.

Our model is unique in that we schedule at minimum, 30 minute appointments, seeing roughly 10-14 patients a day, giving us time to be thorough, deliberate, and precise in diagnosis, and providing in-depth teaching and follow up.

The Innovare approach focuses on achieving and maintaining health by interacting between office visits, leading our patients through specialist, inpatient, and post-acute care.

Primary Duty/Essential Job Functions:

At Innovare, nurses work in close collaboration with our clinicians to systematically assess patient's health status, reach appropriate diagnoses, and implement care plans for effective clinical outcomes in the context of the Innovare Purpose, Clinical Tenets and Vital Signs. The Innovare approach focuses on achieving and maintaining health by providing care between visits and leading our patients and our specialists during inpatient and post-acute care.

More specific job functions relate to:

  • Clinical Judgment and Communication; Organization and Administrative Efficiencies;
  • Triage patient questions/problems; consult with provider and return calls;
  • Assist providers during examinations, perform procedures such as ABI, EKG's;
  • Educate and counsel patients on disease management per protocols, instruct in-home and wound care;
  • Assist in following nursing home cases including incoming nursing home phone calls;
  • Coordinate with providers, IHA referral department and IHA social worker for home health and hospital care to anticipate any medical needs, medications, medical supplies, etc. that may need to be delivered to patient upon discharge;
  • One-on-one contact for managing high clinical risk patients;
  • Record pertinent information in the electronic medical record (EMR) system
