What are the responsibilities and job description for the Coordinator position at InspireAbilities of Anderson County?

JOB PURPOSE

Under general supervision of the Residential Director, coordinates and directs services ensuring quality residential habilitation, residential licensing, supervision, and care of the Friends in the provider setting.

ESSENTIAL DUTIES

1. Supervises and manages facility caseload. Caseload is subject to change in short notice.

2. Coordinates and directs overall service delivery system for the residential setting.

3. Coordinates and follows up on training with In-service Training Department and provide training as necessary. All trainings should have a sign in sheet and agenda to ensure accuracy and quality.

4. T-log Frequency Training definitions and guidelines as required in Residential Habilitation Standards.

5. Complete all required monthly, quarterly and yearly reports as mandated by InspireAbilities of Anderson County and SCDDSN.

6. Serves as on-call person for emergencies in residential facilities and provide coverage, if necessary. Presently, there is on call rotation between ARDs.

7. Evaluate placement to ensure individuals are receiving adequate support in a healthy and safe environment. Placement evaluation is deemed necessary by risk assessment.

8. Meets with supervisors quarterly, or as needed, to discuss and determine needs of the individuals.

9. Visit each facility on a weekly basis, completing a checklist monthly. Checklist will be provided.

10. Assists supervisor with training needs of individuals served required by Residential Habilitation Standards.

11. Complies with budget and works to ensure compliance guidelines.

12. Arrange work schedule to be involved in all shifts.

13. Keep master schedule and records for each home.

14. Must have a working knowledge of residential licensing, and habilitation standards.

15. Must have a working knowledge of individuals, their families, and act as a neutral liaison, advocating for each person served.

16. With assistance of CTH supervisors, will quality assess each facility, at a minimum, on a monthly basis.

17. Ensure that all trainings, teachings, memorandums are communicated at all levels: Direct Support, Supervisor, and Administration.

18. Reviews HRC every 60 days to ensure any restriction in place has been presented for Due Process, per Residential Habilitation. This includes looking at the CTH to determine any restrictions, and ensuring HRC matches every 60 days.

19. Be able to step in as a supervisor, or direct support, in the event of a vacancy.

20. Review, monitor cash-on-hand/purchase cards, and COH ledgers on a monthly basis. Must show signature and reconciliation date, per procedure, and 200-12-DD.

21. Review, monitor, assess each location for final rule and ensure basic assurances.

22. Must be able to utilize provider electronic record keeping system. (Presently Therap).

23. Must be able to utilize DDSN Portal.

24. Monitor effectiveness and completion/facilitation of residential assessments, residential plan (training goals), progress reports, HRC, and financial plans, to include updates and amendments.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Bachelor degree from accredited university in a health/human services, or similar area of study
  • 5 years in supervisory position in the Health and Human Services field- required
  • Valid SC drivers license with good driving record.
  • Computer Skills in the areas of Word, Excel, Internet, and EMR System
  • Ability to work well with others.
  • Ability to coordinate resources internally and externally to meet needs of caseload.
  • Ability to express ideas clearly and concisely.
  • Creative and innovative skills, initiative, motivation, and organizational skills.
  • Ability to work flexible hours.

Physical abilities required include: climbing, balancing, repetitive motion, stooping, bending, standing, walking, running, lifting, grasping, hearing, visual acuity, speaking, writing, pushing, pulling, crouching, crawling, reading, kneeling, reaching, turning, noise tolerance, tolerance and subjection to extreme temperatures, infection. These employees may be subject to verbal, physical or emotional assault by persons served and may be required to wear protective clothing. As a requirement of continued employment, these employees must be able to attend and complete inservice training as dictated by state, federal and board regulation.

Job Type: Full-time

Pay: $21.00 - $22.00 per hour
