Summary/Objective
Care Partner provides routine care in a manner conducive to the comfort and safety of residents in accordance with federal, state and local regulations and within the established policies and procedures of the Community. S(he) is delegated the responsibility in carrying out the assigned duties by the Health & Wellness Director, Executive Director, Charge Nurse or other assigned supervisory personnel to assure that the highest degree of quality compassionate resident care is maintained at all times. Care Partners are customer service experts who interact with leadership, families, vendors, guests, and other associates while displaying exceptional professionalism.
Essential Functions
- Help create an atmosphere of compassion, optimism, warmth, resident interest and positive emphasis, as well as a calm environment throughout the Community
- Assist the Health and Wellness Director in relationships with families. Regularly meets with families regarding any concerns, needs or questions that may arise regarding our residents care.
- Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the Community. Ensures communication, as appropriate, on the results of these visits
- Assists the Health and Wellness Director with department scheduling.
- Cares for resident per the shift assignment determined by Charge Nurse or Health and Wellness Director.
- Responsible for change of shift hand off report to following shifts Care Partners.
- Care Partners who are also certified Med Techs:
- May supervise self-administration of medication and treatments as prescribed by the health care provider; during the med pass, the Care Provider observes residents response to existing treatment plan, is aware of prominent medication interactions and side effects, and observes residents for occurrence of such and notifies supervisor. The Care Partner, if working as a Med Tech, may communicate with pharmacies and or health care providers as necessary to ensure that residents in the Community have medications as ordered and must document accordingly.
- Assist with completion of medication cart audits per policy and direction of supervisor
- Document an accurate accounting of leisure activities, incidents and observations, and any changes in physical or emotional condition of each resident
- Maintain confidentiality and privacy of resident care, procedures and documentation, and communicate with the resident and families on an adult and mature basis with dignity and respect for the residents rights and wishes
- Report any resident abuse, accidents or incidents immediately to supervisor
- Report changes in the residents condition to the charge/staff nurse as soon as possible
- Report all complaints and grievances made by residents, families, visiting individuals, agencies, etc. as well as unauthorized trips the resident makes outside the Community to a supervisor
- Assure that call system is within easy reach at all times and answered promptly
- Assist with caring for residents eyeglasses, hearing aids, prostheses, etc.; Assists in maintaining a physical, social, and psychological environment that is conducive to providing the best interest and welfare of the resident
- Bathe, toilet and dress residents; Assist with Activities of Daily Living (ADL)
- Follow established safety precautions and observe, monitor and intervene or report unsafe conditions in the Community. I.e. checking and reporting lightbulbs when they need to be replaced, changes in room temperature, etc.
- Review Resident Assessment and Plans of Care to residents conditions, etc. and explain each procedure to the resident as care is given
- Follow established universal precautions and isolation procedures and report any communicable or infectious diseases that you may contract
- Keep residents personal articles off of the floor, properly stored, as well as keeping an inventory and assisting to identify residents personal possessions on an on-going basis
- Keep supply room, work areas, and equipment clean
- Assist residents in preparing for and/or engaging in meals
- Assist and participate with residents during activities, and social programs
- Other duties as assigned by management
Work Environment

While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, stooping and crouching all day. The employee must frequently lift or move objects and patients weighing over 50 pounds. The noise level is moderate to quiet. Because the essential functions may require exposure to bodily functions and infectious waste, diseases, odors and other situations associated with health care, including but not limited to exposure to AIDS, HIV and Hepatitis B viruses involving Risk Classification Categories 2 and 3**, environmental and safety conditions will fluctuate. To be qualified for the position of Care Partner, an applicant/employee will need to possess the ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to ones self or others.

Certifications, Licenses, Education and Experience and other Special Requirements
Care Partners may hold Med Tech certifications (optional) or Certified Nurse Assistant (optional) but must have a High School Diploma or equivalent. It is preferred that the Care Partner has six months experience working in a memory care facility, nursing home, hospital, home health agency or other health care facility. S(he) should also possess exceptional communication and customer service skills, both written and verbal as well as strong planning and organizational skills. S(he) should also exhibit exceptional teamwork skills, bedside manner, infection control, and health promotion and maintenance. The utilization of Electronic Medication Record keeping is required.
