Job Description:
The Registered Nurse (RN) with Homecare is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Hospice/Home Health adult and pediatric patients and their families in their home setting. The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home. Utilizing the nursing process, the RN delivers care in compliance with hospice/home health policies and procedures, legal and regulatory standards. Shift Details: Part time/24 hours per week
This is a PAY PER VISIT position, compensated at $39.00 or $45.00 per visit credit dependent upon years of RN work experience.
Mileage reimbursement and shift differentials available

Job Essentials
1. Performs appropriate assessment of physical, social, and psychological status (including cognitive, communicative and developmental skills as appropriate).
2. Uses assessment information and critical thinking skills to collaboratively develop individualized plan of care. Revise and reassess changes as appropriate.
3. Actively seeks patient, family, team, and physician involvement to develop plan of care including needed education.
4. Collaboratively plans and prepares patients for further support needs once the current episode of care outcomes are met (i.e., teaching, referrals, and follow-up).
5. Sets priorities, coordinates, and organizes patient care delivery through effectively managing time, supplies, and resources.
Minimum Qualifications
Current RN License in state of practice. License is verified
Current BLS Certification for healthcare providers
Current driver’s license, insured and reliable transportation and an acceptable driving record
Ability to communicate effectively both verbally and in writing
Ability to transfer patients independently
Note: RNs hired on or after January 1, 2021 who have 12 months or less of working experience as an RN prior to joining Intermountain will be required to obtain their BSN degree on or before June 1, 2026.
Preferred Qualifications
Bachelor's Degree in Nursing (BSN)
Two years working as an RN
Previous experience in an Acute Care, Home Health, or Hospice setting

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of furniture, equipment, power cords on the floor, etc). Need to ascend and descend stairs or uneven surface in order to access patients.
