Description

The Visiting Nurse Association is growing and needing to add one more highly skilled and compassionate Home Health Nurse Case Manager to our team. As a home health nurse case manager, you will be responsible for providing quality healthcare services to patients in their homes. Your role will involve coordinating with other healthcare professionals to develop and implement care plans that meet the unique needs of each patient. There are so many advantages of serving patients in their own home environment. Studies overwhelmingly show that patients prefer to received care in the comforting familiarity of home, rather than moving to a facility or hospital.


Responsibilities:

- Conducting assessments and developing care plans for patients in their homes

- Coordinating with other healthcare professionals to provide comprehensive care to patients

- Providing education and support to patients and their families

- Monitoring and evaluating patient progress, adjusting care plans as needed

- Administering medications and providing skilled nursing care

- Ensuring compliance with all federal and state regulations

Our Nurses:

  • Work day shift M-F with control of daily activities within the schedule. There is a dedicated on call team; however, all our nurses do participate in a limited number of on call shifts each month.
  • Enjoy being autonomous while knowing managers are just a phone call away.
  • Like the challenge of working to the full scope and capacity of practice using complex clinical skills and displaying a high level of confidence to complete professional assessments and monitor new or worsening symptoms
  • Enjoy working with a wide range patient needs based on a variety of acute or chronic illness, recovery from injury or surgery, or are otherwise home bound
  • Want to help patients continue to live independently at home
  • Work closely with all members of the patient’s care team – Home Makers, Aides, Social Workers, Therapists, Physicians, to identify and coordinate care needs
  • Serve as a crucial communication resource for physicians
  • Promote safety and injury prevention solutions
  • Facilitate new client admissions into services
  • Log observations, treatments and other information in our EMR on an iPad
  • Participate with the entire organization to promote confidentiality, quality assessment, and continual performance improvement.

Requirements

- Registered nurse (RN) degree; Bachelor's degree in nursing (BSN) preferred

- Active Iowa nursing license

- 2 Years in home health or case management experience preferred

- Excellent communication and interpersonal skills

- Strong critical thinking and problem-solving skills

- Ability to work independently and as part of a team

- Knowledge of federal and state regulations related to home health careA sincere desire to help others

  • Iowa driver’s license
  • Ability to communicate in English verbally and in writing
  • US work authorization
  • Proof of negative tuberculosis test (we can provide at the start of employment)
  • BLS Certification
  • Mandatory Reporter Training
  • Available to work Monday-Friday 8AM-5PM includes Call & Holiday rotations
