As a Chronic Care Manager, you still provide the same care, support and education to your patients as you've always done at the hospital, office or home health. Only now you're working from home. Our nurses build long-term relationships with their patients. They work with the same patients each month, providing consistent and personalized support that doctor's offices often don't have time for. Our nurses know that patients need this added support, and patient really appreciate the calls from our nurses. That regular engagement helps patients take better care of themselves and keeps them both active and safe longer.
A typical day:
Your typical day as a Chronic Care Manager is filled with checking up on your patients. Has anything changed since you spoke with the patient last time? Did they see the podiatrist that you helped schedule the month before? Do the new instructions from the doctor make sense? You get to see the real personalities of your patients and never know what the next conversation will bring. You also have opportunities to establish relationships new patients and build trust and credibility with them and their families.
You are a key part of the patient's care team and communicate directly with providers and the in-office staff. Using your own expertise and following the provider's care plan, you keep patients on task – prioritizing their own good health, verbalizing their understanding of their conditions and getting preventative service. You are also an advocate for your patients, finding answers to questions and helping resolve their medical challenges quickly and efficiently.
You use technology tools to help you manage your patients, gather educational resources, coordinate patient care, and maintain each patient's care goals. With your experience in Electronic Medical Record (EMR) systems, you review recent office visits, communicate with the care team, and then accurately document what you've done on behalf of your patients.
Position Responsibilities
- Provides nursing care according to relevant Board of Nursing Scope of Practice Decision Making Model, the relevant Nurse Practice Act, and Lynk Health’s policies, procedures, and protocols.
- Communicates with patients enrolled in in value-based programs by providing information that is clinically relevant and empowers patients to make appropriate decisions, including the decision to self-care.
- Monitors patient data using digital means.
- Accurately and completely documents clinical and other relevant notes within the electronic health record in a timely manner throughout and immediately after the call.
- Liaises effectively between patient and third party in critical situations, such as need for ambulance transport, police intervention, suicidal, or other situations in which information is not consistent or may be disputed.
- Maintains on-going communication with provider and practice staff.
- Adheres to time utilization guidelines and ensures that personal workflow supports the service to better meet the demand for Telehealth services.
- Performs other related duties as assigned.
Minimum Qualifications
Hold a Certificate or Diploma from a state approved licensed practical nurse training program and hold an unencumbered, multistate nursing license. Also, the incumbent must have a reliable, wired internet connection and appropriate home “meeting” space.
Preferred Qualifications
- Two (2) years of clinical practice experience including use of EHR/EMR software.
- Working knowledge of current nursing intervention procedures.
- Geriatric Care / Primary Care experience
- Excellent communication, telephone, and customer relation skills.
- Ability to work independently on assigned tasks.
- Ability to build long-term relationships with patients and providers.

Pay: $20.00 - $22.00 per hour
