The Care Coordinator serves an integral role by acting as a liaison between the pharmacy team and a partner facility. These duties include but are not limited to building strong relationships with clinic partners and patients, assisting with coordination of medication refills, insurance barriers, and the full services of the Janus RX pharmacy. This is a great opportunity to utilize the skillset of a pharmacy technician in a non-traditional environment. A key function will be promoting Janus RX services to onboard and retain consumers and patients. A successful care coordinator will have strong communication skills and be able to perform detailed work.
The person will be working at the Boyne Falls, MI location. This position will require regular travel to other locations of the agency as well. There will be mileage reimbursement for any travel to sites.
Job Duties
Build strong relationships with various Janus RX partners including consumers, case managers, and clinic staff to educate and deliver on the pharmacy services
Drive fun and exciting marketing events to promote Janus RX services in partnership with the clinic
Enroll consumers in our services and aids in continuity of care by coordinating prescription refills between providers’ offices and subsequent delivery of medication to the patient
Working directly with providers’ offices and/or facilities as part of an interdisciplinary team-based approach to medication management
Inventory management as necessary by the facility partner
Advocates for patient health by navigating common barriers such as prior authorizations, patient assistant programs, sync fills, or delivery
This position has access to and knowledge of extremely sensitive, private, and confidential materials. Ability to maintain the highest standard in this area is required.
Minimum Employment Requirement(s)
Possess and maintain a valid professional license/certification/registration issued by the State Board of Pharmacy
High school diploma or GED
Preferred Qualification(s)
Pharmacy Technician Certification Board (PTCB) certification One year of experience as a pharmacy technician OR
Two years of healthcare experience prior to hire
