EXPERIENCE: One to two years’ experience in a home health agency or health care setting preferred. Scheduling experience preferred.
EDUCATION & TRAINING: High School diploma or equivalent required. Associate Degree preferred.
OTHER REQUIREMENTS: Excellent Data Entry skills, proficient in Windows environment, knowledge of spreadsheet, WP, strong analytical skills required. Ability to communicate with all levels of home care personnel.
PRIMARY FUNCTION:
Performs under the direct supervision of a Home Care Clinical Nurse Manager or Associate Director. Performs computer related functions required for scheduling and clinical information support. May also have the responsibility of performing a variety of functions that support departmental administrative activities.
ESSENTIAL FUNCTIONS:
- Meets all standards of the Patient Satisfaction Initiative Criteria in order to make Abington – Jefferson Health (AJH) the hospital of choice for Patients and Employees.
- Utilizes the computerized electronic scheduling package to assign patients according to clinical manager guidance, patient need and staff availability.
- Performs data entry activities in order to process patient information necessary for patient care and meet medical record requirements.
- Collaborates with clinical team members to ensure productivity standards and flow of clinical information needs are met.
- Reviews pertinent reports to ensure accurate data and patient information is maintained in accordance with departmental procedures.
- Completes documentation in accordance with department standards.
- Performs other duties as assigned.
