The Part Time Community Based Adult Services (CBAS) Registered Nurse (RN) conducts eligibility assessments using the CBAS Eligibility Determination Tool (CEDT) for referred LA Care Health Plan members. The CBAS center attendance is to promote living independently for those that pose a potential risk for placement in a Skilled Nursing Facility. The RN will conduct assessment to gather information by reading medical records and speaking with referred clients, client’s family member(s), and/or staff members working in the CBAS center. Topics of assessment include: diagnoses, medications, assistive/sensory devices, systems review, ADLs/IADLs (Activities of Daily Living and Instrumental Activities of Daily Living or basic self-care tasks). S/he is responsible for performing face-to-face assessments with older adults and/or adults with disabilities in the community. S/he will gather information to determine whether the individual meets the medical guidelines to attend the CBAS Center for day services. The RN will assess the client as a whole individual and be responsible for following up on any crisis related issues or medical concerns as needed.
