Job Details

Home Care Field / Visiting RN seeing patients in their homes M-F 5x per week. Manhattan territory

What you will be doing

• Complete initial assessments, OASIS, recertifications
• Creates and implements a plan of care
• Collaborates with the interdisciplinary team and coordinates patient care
• Educates patients and families
  
  

Skills you need

• RN license in NY
• Home Care Experienced Required as a RN, LPN, CNA, or HHA.
