Summary:
The Rehab Liaison:
Works directly with the multidisciplinary care team overseeing each patient’s care to facilitate timely and effective admission to the ACIR units at Johns Hopkins Bayview Medical Center (JHBMC) and The Johns Hopkins Hospital (JHH). The multidisciplinary team includes but is not limited to: financial clearance, admissions, physicians, advanced practice providers, rehabilitation department staff, care coordination/case management, and ancillary/support staff. The incumbent will work closely with patients and caregivers to ensure adequate provision of comprehensive rehabilitation services are made available in the hospital environment.
Education:
- Position requires the knowledge of theories, principles, and concepts typically acquired through the completion of a Bachelor's degree in Nursing, Master's degree in Occupational Therapy, Physical Therapy or Speech Language Pathology, current BLS, and three years of clinically relevant experience.
Required Licensure, Certification, Etc.
- Licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, Speech Language Pathologist
