Description
The Medical Records Clerk is responsible for the assembling, indexing and analyzing of health records of JHI according to state and federal regulations, as well as HIPAA and HITECH legislation. The clerk will also work with HIM Coordinator, Director of Privacy, and Director of Security to respond to all release of information request in an ethical, timely, compliant and secure manner. All Juniper Health employees will adhere to the Patient Center Medical Home (PCMH) model and work to ensure that patients of all ages receive comprehensive, coordinated and patient-centered whole patient care. The employee will maintain confidentiality at all times.
DUTIES & RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
Assemble, index, and analyze the health records of Juniper Health
Respond to all Release of Information requests in a timely, compliant, and secure manner
Verify accuracy of patient intake documentation on a continual basis
Answer incoming medical record requests calls, document the requests, and follow through in the appropriate manner.
Indexes paper records in either an electronic records system or file system as appropriate
Document all medical record requests properly within the electronic health records and electronic dental records systems.
Maintain confidentiality of all patient records encountered
Track and complete all requests according to requestors guidelines and timeframes as they relate to the necessary laws, legislation, and policies.
Audit electronic health records for continued accuracy.
Monitors incoming fax and email queues to ensure timely delivery of documents
Perform other job-related duties, as may be assigned.
Qualifications
QUALIFICATIONS:
Minimum of (3) years medical records/registration experience desired.
Knowledge and experience in state and federal information security laws, including but not limited to HIPAA.
Demonstrated organization, facilitation, written and oral communication, and presentation skills.
Possesses a high degree of accuracy and attention to detail
A clinical background is a plus
WORK ENVIRONMENT:
Work is typically performed in a clinical office environment.

While performing the duties of this job, the employee is required to communicate, operate equipment, observe details at a close range, be in stationary or moving positions, and occasionally lift or move up to 25 pounds. TRAVEL:
Travel to JHI sites and/or external meetings and trainings may be required.
OTHER:
The employee is expected to work and be available during regular business hours, however, extended hours and weekend work may be required.
