What are the responsibilities and job description for the Peer Advocate position at JWCH Institute?
Position Purpose:
The position participates as a member of a multi-disciplinary team providing health and social services to clients of the Wesley Health Centers. In addition to participating in a multi-disciplinary team, the position participates with a health education and prevention team. The position delivers skills-building information and personal life experiences with similar life challenges to clients of the Wesley Health Centers.
Principal Responsibilities:
- Establish a rapport with clients and express a concern for their well-being.
- Provides support to clients and motivate individuals to adhere to behavior modification program.
- Maintains complete updated, confidential file on the status of the client and document their progress in changing personal behaviors.
- Contact client once to twice per month during regularly scheduled hours.
- Accompany client to health and/or social services referral, encourage compliance, and document progress on appropriate forms.
- Make referral to substance abuse recovery services, health care facilitates, family planning services, housing and social service organizations.
- Deliver education and skills-building information as presented in approved curriculum.
- Share personal life experiences with incarceration, substance abuse, mental health, and other risk behaviors.
- Assist in collecting program data and processing program forms on an ongoing basis.
- Assist in preparing monthly reports to funding agency’s.
- Performs other duties as assigned.
Requirements:
- Previous experience working with homeless persons.
- High School diploma or GED.
- Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness.
- Personal life experience in overcoming challenges similar to the client.
- Ability to travel and arrive to sites on time within the community.
- Desire to work with homeless persons.
