What are the responsibilities and job description for the RN, Heart Failure position at Kaiser Permanente?
Under the supervision of onsite physician, performs various professional and administrative duties related to heart failure including but not limited to: patient preparation, managing schedule of heart failure members in clinic, application of ACLS measures to patients when needed. Specifically, be responsible for managing outpatient Heart Failure Pathway; scheduling appointments, patient interviews, physical assessments, patient consent and signatures, and conducting patient teaching. RN will assist is coordinating care with Cardiologist, Clinical Pharmacist, and Nutritionist in the process of adjusting cardiac medications.
Essential Responsibilities:
- Manage Heart Failure Pathway: Referrals ordered by Cardiologists; series of Nurse Visit appointments for heart failure members over a 6-month period; Goals- Achieve target doses or maximally tolerated dose of heart failure medications. Improve patient CHF education. Decrease heart failure associated hospitalizations.
- Close and frequent communication with CHF team: Cardiologist, Pharmacist, Nutritionist, Case Management.
- Direct point of contact for CHF members seen by Cardiologist.
- Provide patient education on self-management skills: fluid restriction, sodium intake, weight management, and medication adherence.
- Follow up phone calls and call backs with members and interdisciplinary team.
- Clear and concise documentation of assessments and follow up information.
- Monthly Meetings with CHF team.
- Assist with Heart Failure University; series of workshops available to all patients and family members who are dealing with heart failure.
- Monitor 30 day, 90 day and 6 month readmissions rates in patients enrolled in the pathway.
- Assist back office staff but not limited to: IV insertions, blood pressure checks, support Stress RNs with stress testing.
- Performs other related duties as required.
Basic Qualifications:
Experience
- Minimum three (3) years of RN clinical nursing experience or have completed a Kaiser Permanente Nurse Residency program in Georgia.
- Minimum two (2) years in Heart Failure Clinical Management.
Education
- Associates Degree or two (2) years of experience in a directly related field.
- High School Diploma or General Education Development (GED) required.
License, Certification, Registration
- Registered Professional Nurse License (Georgia) required at hire
- Basic Life Support required at hire
- Advanced Cardiac Life Support required at hire
Additional Requirements:
- Strong medical and/or surgical and heart failure experience.
- Complete a customer Service Assessment.
Preferred Qualifications:
- Bachelor of Science in Nursing.
