What are the responsibilities and job description for the Patient Navigator position at Katahdin Valley Health Center?
POSITION SUMMARY
A Patient Navigator is an individual trained to help identify and resolve real and perceived barriers to care, enabling patients to adhere to care recommendations and thus improve their cancer outcomes.
Serves as a point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Katahdin Valley Health
Center and the community. The Patient Navigator is responsible for providing health education targeting patients’ needs and health risk factors, assisting patients to schedule and complete cancer screenings, facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians. The Patient Navigator is responsible for helping patients and their families navigate the healthcare system and community-based services.
RESPONSIBILITIES
1. Provide patient outreach and health education focused on the importance of cancer screenings.
2. Responsible to identify and increase the appropriate cancer screenings needed for KVHC patients.
3. Review screening results to identify next steps and appropriate action.
4. Responsible for monitoring and addressing appointment gaps throughout the care continuum.
5. Work collaboratively with the referral team to close the loop on all orders.
6. If a patient has a cancer diagnosis work collaboratively with the care management team to identify and address patient needs.
7. Partner with patients and their families to help navigate the healthcare system and community- based services targeting their health and social needs.
8. Follow patients throughout the care continuum, including inpatient admissions. Collaborate with
Care Management resources and external partners in the community to simplify and ensure a seamless process for the patient.
9. Serve as a patient advocate and liaison.
10. Provide guidance and education to patients who identify cost as a barrier to care.
11. Continuously expand knowledge and understanding of resources, services and programs available to assist patients with identified barriers.
12. Develop and enhance patients understanding and skills to effectively communicate with healthcare providers and organizations.
13. Maintain and update spreadsheet of identified activities for each type of cancer screening.
14. Assists with development of patient education programs and tools.
15. Performs additional duties or projects as assigned by the Clinical Practice Director.
16. Adheres to all KVHC policies and protocols.
17. Complies with all safety rules and protocols. Reports all incidents, regardless of severity, immediately to supervisor and completes an incident reports and investigation form, which will be forwarded to Human Resources within 24 hours of the incident.
18. Be a good steward in the communities serviced by KVHC; represent the organization at outreach activities as needed.
