What are the responsibilities and job description for the LPN position at Kelsch Associates?
Agency LPN is responsible for supporting Kelsch Associates' medical department to ensure the department runs smoothly. The LPN is responsible for administrative tasks, such as maintaining individual records, reviewing case notes and other accompanying paperwork, preparing documents, assisting current agency nurse with tasks as needed.
Requirements:
- Active practitioner's license (LPN)
- Caring personality
- Experience with adults with Intellectual Developmental Disabilities a plus, but not a requirement.
- Must be willing to travel to our group homes located throughout Chester County, ie. Phoenixville, Downingtown, Coatesville, Malvern, Parkesburg, and West Chester
- Strong verbal, written and computer skills essential
- Driver's License
Work schedule:
40hrs/week daytime position 8:30-4:30 Monday - Friday. No weekends. No holidays.
Duties:
- Working independently and as part of a team to meet the individual's needs
- Review and file all required individual and program documentation and reports, ie. case notes, labs, seizure charts, MARs, sleep charts.
- Work in conjunction with current Agency Nurse and Program Directors when needed to aid in hospital discharges, calls from physicians and communicating with pharmacies.
- Review and keep up to date electronic health records and individual binders.
- Record individual medical history, vital statistics and test results in medical files.
- Support training compliance
- Facilitating hospital admissions and discharges to ensure the health and safety of individuals.
- Ensure staff and individual's safety by training staff in the correct use of program medical equipment, transfers, care and medical treatments as prescribed following agency philosophy, protocol and licensing standards.
- Reports all changes to administrative staff and/or primary physician regarding individual's physical and psychological conditions.
- Ensures all adaptive equipment is present and in good working condition. Follows up on needed repairs.
- Attends medical appointments for newly diagnosed, chronic conditions and complex medical issues as a patient advocate.
- Performs treatment, care and training within the scope of certification
- Becomes familiar with and follows agency philosophy, policy and procedures and residential group home licensing regulations.
- Review of individual's medication
