What are the responsibilities and job description for the Medical Records Clerk position at Kent Regency?
POSITION SUMMARY: The Medical Records Clerk provides daily medical records management, including document collection, technical validation and records maintenance to ensure compliance with HCFA and other regulatory agencies.
RESPONSIBILITIES/ACCOUNTABILITIES: 1. Processes daily medical records image documentation as uploaded by facility operations, in a timely and accurate fashion. 2. Ensures that daily image processing is recorded through the Medical Records Master tracking spreadsheet in real time, to assist in the daily assignment of staff workflow. 3. Communicates daily with operational staff (DOR/ROC/COAD) once their facility has been processed to rectify rejected records. 4. Distributes out of compliance communication (60 day) to operations to assist in the resolution of document issues delaying claims billing. 5. Coordinates out of compliance conference calls (90 day) with operations to assist in the resolution of document issues delaying claims billing. 6. Review claims hold reporting to assist in the resolution of missing image data (signature, documentation, dates of treatment, and pending discharge or evaluation orders). 7. Coordinates communication to site operational staff (DOR/ROC) regarding missing signatures and documents. 8. Coordinates Records Inventory Management Report (RIMR) maintenance in coordination with site operational staff (DOR/ROC) regarding verifying the necessity for removing records via write-off for patients that were not treated or discharged but COM still created a record. 9. Answers day-to-day questions from Directors of Rehab and/or Rehab Tech. 10. Performs other tasks as assigned.

Qualifications: SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1. The Medical Records Clerk position must possess a minimum of a high school degree or equivalent. Experience in claims, billing and medical records is preferred. 2. Job requirements also include strong organizational and communications skills. Along with being detailed oriented, have data entry skills along with Word and Excel knowledge. Google Sheets and Docs experience preferred. 3.
