About Korean Community Center of the East Bay
Since 1977, Korean Community Center of the East Bay (KCCEB) has been uniquely positioned to serve the Asian community in the Bay Area. After the 1965 Immigration Act which allowed for an influx of immigrants from Asia, KCCEB was one of five Bay Area organizations developed in the 1970s to support the resettlement and community health of Asian immigrants and refugees. KCCEB has evolved into a unique entry point for Korean and other immigrant and undocumented children, youth, adults, families and seniors who have nowhere else to turn to for their essential support, including health access, social and legal counseling, case management and mental health wellness.
The Programs
KCCEB is currently composed of three divisions namely the Immigration Integration Program, the Community Health Access Program, and the Community Wellness Program. The Immigration Integration Program creates access to legally authorized non-low-cost immigration services that are needed for immigrants to transition into American life and become engaged members of society. The Community Health Access Program creates access to affordable health care and social services including healthcare enrollment and literacy education, social service support, case management, and health promotion to seniors, a Korean language information and referral service, and community capacity building through the development of health advocates. The Community Wellness Program is a newly established program addressing the mental health and substance use prevention needs of Asians in North Alameda County. It raises mental health awareness, reduces mental health stigma with attention to cultural affinities and values of monolingual low-income East Asians (e.g., Korean, Chinese, Japanese, and Mongolian).
The Position
This Community Program Coordinator will provide case management and social services support to API families, adults, and seniors in Alameda County. This is a unique opportunity to integrate individual case management and wellness services that provides holistic and wrap-around support to immigrants and refugees in the API communities.
Responsibilities: Case Management and Wellness Program (50%)
● Provide comprehensive case management services, social services follow-ups, and wellness support to API adults and seniors
● Conduct comprehensive bio-psycho-social assessments and intakes with the development and implementation of individualized care plans with assigned clients
● Conduct outreach and in-reach, including home and community visits, to assess or recruit program participants and build connections with community-based partners (i.e. community-based organizations, faith-based organizations, senior centers and/or housing, etc.) for capacity building;
● Assist in the development and maintenance of the community resources (i.e. legal, housing, local social services, mental health, etc.) to remain current regarding information and services available to address clients’ need
● Conduct wellness screenings for seniors residing in the Alameda County to assess general health, mental health, safety, and services needs
● Ensure client-level data be entered into the Social Assistance Management System (SAMS),
● Maintain client files in accordance with legal and grant requirements, and complete all documentation in a timely manner
● Maintain client caseloads of 25-35 per annual basis
● Receive training to become Covered CA certified enrollment counselor in the first year of employment
● Assist other KCCEB programs as needed as directed by Program Supervisor
Responsibilities: Community Health Access Program (50%)
● Be the main enrollment assister for Medi-cal, CalFresh enrollment, renewal application, and other social services needs for the API community, with focus on limited English proficient immigrant individuals and families
● Assess clients to determine eligibility for Medi-cal and CalFresh programs, as well as other public benefit programs that are available. If clients are eligible for other public benefit and/or social services programs, provide application assistance or connect them with other staff that can provide the assistance
● Assist with Bay Area Korean Infoline (BAKI) coverage, organization’s initial contact for social services, resource navigation support, and information & referral
● Provide appropriate referral and/or resource for those who do not require or who cannot be accommodated by in-house services
● Receive training to become Covered CA certified enrollment counselor in the first year of employment
● Document and record all social services including Medi-cal and CalFresh cases that were initiated, processed, and completed into monthly reporting database, case logs, and individual client files in accordance with grant requirements and in a timely manner
● Ensure that client data and services information are entered and updated on SalesForce
● Maintain up-to-date information on public benefit programs including Medicare, Medi-cal, CalFresh, Social Security, as well as other community resources and share this knowledge with other staff who provide direct services
● Assist other KCCEB programs as needed as directed by Program Supervisor
Qualifications:
● Bachelor’s (BA) Degree in psychology, community health education, public health with 1-2 years of experience, or Associate of Arts (AA) degree with 4 years experiences in community-based programs or mental health services
● Bi-lingual in English and Korean strongly preferred
● Understanding of the needs of immigrant and refugee populations with limited English proficiency;
● Personal qualities of integrity, credibility, and a commitment to and passion for KCCEB’s mission;
● Experience in governmental funding including Alameda County Social Services and/or Behavioral Health Care Agency highly desired
● Must be a dynamic self-starter with initiative, ability to manage time and workload with a sense of inspiration, collaboration and humor, especially when working with clients under high stress situations
● A strong team-player with ability to work collaboratively within the team as well as with other partner organizations and communities
● Strong communication skills, including diplomacy and ability to ensure confidentiality
● Strong familiarity of mental health, substance abuse, and other culturally related issues, especially as these impact un and underserved API communities
● Interest in or knowledge of traditional health and wellness practices and perspectives
● Computer and database literacy
● Valid California Driver’s License, clean driving record and insurable
Compensation and benefits
Salary range: $58k-$62K commensurate with background and experience.
