What are the responsibilities and job description for the Service Facilitator position at La Casa, Incorporated?
The Service Facilitator’s position is a Part-Time Independent Contractor (1099) Position. This position to provide ongoing supervision of Individuals with disabilities receiving services through their insurance or a Medicaid Waiver from (DMAS) the Department of Medical Assistance Services in Virginia by conducting monthly home visits. The Plan/position requirements prefer that the Service Facilitator/Independent Contractor, possess a minimum of an undergraduate degree in a human services field or be a RN currently licensed to practice in Virginia. It is preferred that the Service Facilitator have at least two years of satisfactory experience in the human services field working with persons with disabilities or the elderly. The Facilitator must possess a combination of work experience and relevant education that indicates possession of the above suggested knowledge and skills and live in the area/counties to be served. Areas to be covered are Axton, Danville, Brookneal, Keeling, Nathalie, Ridgeway, Rustburg, South Boston and Vernon Hill. ** Previous applicants need not apply.
