Overview
Leo Human Services LLC is currently hiring for a Housing Case Manager who specializes in marketing, keeping up contact with case managers, keeping up with coordinator's notes and work, intake on new clients, and managing coordinators. This position is vital for the company and requires someone who has experience being a housing coordinator and housing clients.
Duties include:
- Establish supportive and motivational relationships with clients.
- Review and monitor services provided to clients.
- Connect clients to community resources and programs.
- Maintain client records in electronic health record system.
- Perform intake on new clients
- Manage coordinators notes
- Connect with other companies and case managers as a marketer
- Collaborate with other family members, service providers, staff members, consultants, interns and/or volunteers.
Qualifications:
- Bachelor’s degree in related field preferred.
- Minimum 2 years of housing coordinator experience.
- Ability at problem solving
- Ability to work independently and collaboratively
- Experience with social media networking and outreach
- Excellent verbal and written communication skills
- Must be proficient in Microsoft Office products including Word, Excel and PowerPoint.
- Must be able to work both independently and on a team.
- Must have leadership qualities
- Knowledge of the Minnesota housing challenges and resources
- Working knowledge of and experience with navigating the housing systems
- Experience navigating database software and platforms including but not limited to HMIS and Coordinate Entry
- Experience providing services through a trauma informed care and harm reduction framework
