Position Title: Community Based Direct Support Professional
Office / Field Based: Community Based
Compensation: $15.00/hour plus a $10.00 a day gas stipend.

Work Schedule: Mon-Fri 8:30AM-2:30PM
Job Description / Typical Duties: Interacting with assigned individual out in the community, assisting with volunteering positions, setting up different activities of the individual’s interest and promoting independence in the community.
Client Description:
Client is a female in her 20’s living in the Rochester area. She has Intellectual disabilities and a behavior plan. She needs assistance with transportation to volunteer sites as well as stand-by assistance. She is looking to find meaningful employment in her area. She enjoys shopping, animals, socializing with peers, going to the gym and other activities.
Required Education / Skills: High school or equivalent.
Typical DSP Responsibilities and Duties:
- Support individual with intellectual and developmental disabilities and/or traumatic brain injuries in integrated community settings.
- Tailor services and daily activities with consideration of the individual’s needs, interests, competencies and lifestyle as outlined in the individual service plan.
- Provide structure and assistance to help the individual learn, improve and maintain basic living skills, personal decision-making skills, social skills in the community, safety skills at home and community, rights and responsibilities and a healthy lifestyle.
- Assist the individual in securing volunteer and employment opportunities.
- Provide opportunities to become aware of community services and resources.
- Provide transportation to and from activities throughout the day.
- Complete daily notes and incident reports as assigned.
- Attend individual service plan meetings, team meetings and medical appointments with the individual as assigned.
- Perform duties to reflect agency policies and procedures.
- DSP must be aware of and document who they have been trained with and provide coverage for only such programs. It is essential that no staff work with an Individual whom they are not sufficiently trained with.
- If the Individual supported has a behavior plan, staff must be trained in such plan.
- Always be punctual for pick up and drop offs. Report to the first pickup five minutes prior to the start time, in order to avoid tardiness.
- Responsible for following activity schedule if changes to the schedule must take place advance notice to supervisor must occur. Ensure that activities of which Individuals are participating in are in correlation with their goals, are age appropriate and are billable services.
- Ensure that HIPPA laws and regulations are always followed.
- Thoughtfully participate in at least one evaluation per year.
Special Work Conditions: Knowledge/Skills/Experience Required:
· Willingness to be trained in CPR, First Aid Certification, MANDT & Medication Administration.
· A valid driver’s license, reliable transportation, clean driving record and auto insurance is required.
· Position requires both local and national criminal background check
· Access to Internet
Education Required:
· High School Diploma or equivalent required.
· Must be 18 years old or older
