GENERAL DESCRIPTION
The Care Coordination Supervisor is responsible for assignment of services within the Living Room and in the community. Core task is to ensure that services provided are in accordance with all state and grant reporting requirements and to create a seamless transition for individuals receiving services across a full spectrum of health and social services, so that all gaps in care are reconciled within a trauma-informed, person-centered model of care. Supervisor serves as the expert of all service delivery and can train, model, and oversee all service delivery at the Living Room and in the community. This position is responsible for linking individuals to appropriate natural and community resources to meet ongoing needs and assisting them to navigate appropriate service options available. Ensure that individuals receive timely access to quality care, working with LifePath crisis team, contracted facilities, and other community organizations to identify needs and reduce crisis episodes and help individuals in services establish a primary care relationship in order to address their medical needs.
Supervisor must provide individual and/or group coaching/supervision regularly to ensure that staff fully understand their clinical expectations and are maintaining service provision and documentation standards. Coaching/supervision will consist of observation, regular chart audits including monthly fidelity reviews, and regular discussion with team members on areas in which they need additional clinical guidance. Supervisor is also responsible for coordinating interdisciplinary team meetings as needed for those being served at the Living Room. Supervisor will ensure that NOMS are completed following grant standards and complete all required data collection and team reports.
Supervisor will maintain a regular caseload and cover team member’s caseloads if the team is short a member or someone is out on leave and will provide full-service array as allowed by credentials. Supervisor will also be responsible for overseeing scheduling of staff, approving paid time off and bi-weekly timesheet approval of team members. Supervisor works under the supervision of the Program Administrator for the Living Room and ensures effective communication regarding provider operations, performance, and problems.
EDUCATION, TRAINING, & EXPERIENCE
- Must have a Bachelor’s degree in psychology, social work, nursing or related field from a four-year college or university that meets the Texas Administrative Code for requirements for a Qualified Mental Health Professional- Clinical Services (QMHP-CS).
- Master’s degree in counseling, social work, or related fields with unrestricted license as LPC, LCSW, or LMFT preferred
- Must have at least three years of clinical experience in the treatment of individuals with mental illness and/or substance abuse disorders.
- 2 years leadership or supervisory experience highly preferred.
ESSENTIAL TASKS & STANDARDS
- Ensure individuals receiving services get timely access to quality care. This includes and is not limited to assessment of needs, referrals completed, and follow-up.
- Participate in interdisciplinary treatment teams for individuals on caseload to identify gaps in services and opportunities for collaboration.

- Ability to foster collaborative relationships with key interfacing teams, be a self-starter who works closely with all team members and takes initiative in completing tasks.
- Ability to ensure effective communication with the multi-disciplinary team and across a variety of community resources.
- Will work under general supervision with moderate latitude for the use of initiative and independent judgement as well as identifying when a licensed professional needs to be consulted.
- Knowledge of the social services system with the ability to connect individuals with appropriate services.
- Must be organized, detail oriented, computer proficient, and able to write clearly and professionally.
- Ability to respond with individuals and staff with patience, empathy, and understanding.
- Work hours may include evening and weekend hours as program develops to meet the needs of the community. Flexibility in schedule required.
- Have ability to multitask and be able to provide exceptional customer service in a busy working environment.
CERTIFICATES, LICENSES, & REGISTRATIONS
- Must have valid Texas Driver License and a good driving record.
- Must be able to pass a criminal background check.
- Must be able to complete all LifePath Systems training.
- Must be able to provide at least 3 professional or educational references.
BENEFITS
Salary Range: $ 58,000-70,000 per year (salary depends on education, experience and license)
Shift Hours: Monday - Friday with shift hours between 7:00am - 7:00pm (Hours will vary, flexibility required to meet demands of program)
