GENERAL DESCRIPTION
Performs routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care model. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Provides person directed skills training and psychosocial rehab to develop coping and other skills needed in order to feel successful in their home and community. Requires use of initiative and independent judgment in order to provide interventions in the community, home and school to individuals on caseload. Is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the plan for services to meet the individual’s identified needs. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Must have the ability to work independently and collaboratively as needed and have effective communication with supervisory staff when necessary. Expectations include timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage clinic flow, adhere to crisis procedures and be available to assist with crisis and other coverage areas as needed.
ESSENTIAL DUTIES, RESPONSIBILITIES
- Provides direct services to individuals on caseload including completion of uniform assessments, person centered recovery planning, and all services authorized by level of care.

Provide Psychiatric Rehabilitative Services (individual &/or groups) as described in the Texas Administrative Code.
Services include:
- Routine Case Management
- Skills Training and Development
- Medication Education / Patient and Family Education
- Psychosocial Rehabilitation
- Documentation
- Understands and implements trauma informed care practices.
- Demonstrates effective problem solving skills.
- Teamwork/Cooperation
- Appropriate Use of LifePath Systems Equipment and Materials
- Provide crisis services and problem solving assistance to staff and individuals in BH program.
EDUCATION and/or EXPERIENCE
- Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
- At least two years of related experience and/or training in the field of mental health preferred.
KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
- Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to write clearly and professionally.
- Ability to think clearly and logically in order to deal with potentially dangerous and/or emotional situations.
CERTIFICATES, LICENSES, REGISTRATIONS
- Able to complete all LifePath Systems required training.
- Must have valid Texas Drivers License and good driving record.
- Must be able to pass criminal history check.
- Must be able to provide at least three (3) professional or educational references.
BENEFITS
Salary: $40,000 - $45,000 per year
Shift: Monday - Friday (Hours will vary to meet the needs of the individuals in services)
