Nurse Practitioners arekeymembersof LifesparkHealth's professional medical practice.Wedeliver primary care and consultative servicesto frail and complex patient populations.This includeshome-basedprimarycare in the community and on senior housingcampuses and consultative care in transitional care units. We utilize a combination of in-person and telehealth visits inside a multidisciplinary team-based model. Patients and their caregivers receive expert, accessible, and compassionate care.
This collaborative and dynamic medical practice is led by industry experts in geriatrics and senior care services. It is delivered under the umbrella of a larger Lifespark enterprise which provides whole person senior care to broad populations of seniors. Lifespark has value-based arrangements with payers which allows clinicians to practice independent of traditional healthcare and payment constraints. Qualifications
- A currentMinnesotaAPRN and RNlicenseand DEAlicense
- Certification in Geriatrics preferred
- Board certificationthroughANCC or AANP
- Meet threshold criteria/qualifications for Credentialing and Privileges
- Communicates effectively in verbal, written, and electronic methods
- Able to work in non-traditional settings (client homes, nursing homes, and from home)
