Wellness Nurse-LPN

Job Summary:
We are seeking a dedicated and compassionate Wellness Nurse to join our Assisted Living team. As a Wellness Nurse, you will play a vital role in providing quality healthcare services to our residents. Your primary responsibility will be to ensure the overall well-being and health of our residents by administering medications, conducting physical examinations, and providing general nursing care.

Duties:
- Administer medications and treatments as prescribed by physicians
- Conduct physical examinations and assess residents' health status
- Monitor vital signs and document any changes or abnormalities
- Assist with the management of chronic conditions such as diabetes, hypertension, and respiratory disorders
- Provide wound care and perform dressing changes as needed
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Assist with activities of daily living (ADLs) for residents who require assistance
- Provide dementia care and support for residents with memory impairments
- Educate residents and their families on wellness strategies and disease prevention
- Maintain accurate and up-to-date medical records

Qualifications:
- Valid nursing license (LPN)
- Previous experience in a hospital or long-term care setting is preferred
- Proficiency in performing physical examinations, venipuncture, and administering medications
- Knowledge of DME (Durable Medical Equipment) usage and maintenance
- Familiarity with dementia care techniques and interventions
- Strong communication skills, both verbal and written
- Ability to work effectively in a team environment

Pay: $29.00 - $35.00 per hour
