JOB FUNCTIONS

Provides mandated case management services to eligible individuals as defined in OAR 411-320-0090(4) which include but are not limited to:

  • Development of individual support plans and other required documentation. 
  • Conducting needs and risk assessments to determine service needs.
  • Authorize and monitor services including residential, in-home, and employment services.
  • Provide information, referrals and coordination to access needed services.
  • Facilitate all entry, exit and transfers including home and community-based services, the Oregon State Hospital system, State and local correctional facilities, nursing services, support brokerage services.
  • Take lead in providing for crisis and protective services intervention.
  • Conduct or participates in licensing and certification processes for service providers in conjunction with State and County licensing staff.

Non-essential functions:

  • Participate to the extent resources allow in Individualized Education Program (IEP) planning meetings and transition programs. 
  • Participate in assessment of community needs, outreach, and planning activities and events.
  • Attend DD advisory council meetings as needed.
  • May be delegated civil commitment activities by the DD Program Manager.
  • Perform other related duties as assigned. This document in no way implies that there are the only duties to be performed by the employee occupying this position.

Minimum Education Level

Bachelors Degree

Specifics:  In behavioral science, social science or a closely related field.

 

Minimum Experience Level

One year 

Specifics:  Experience in a related human services field.

 

Substitution Note: A bachelor’s degree in any field and one year of human services related experience; An associate’s degree in behavioral science, social science, or a closely related field and two years of human services related experience; or three years of human services related experience.

 

Driver’s License

Oregon Driver's License required

Specifics:  Ability to obtain within 30 days of appointment.

 

 

Desirable Qualifications: Bilingual and/or bicultural competencies for Latino population.

KNOWLEDGE, SKILLS AND/OR ABILITIES

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The required knowledge, skills and/or abilities are listed below:

Knowledge of:

  • Applicable Oregon Administrative Rules.
  • The public service system for developmental disabilities.
  • Person centered practices necessary to effectively work with persons with intellectual and developmental disabilities.

 

Skill or ability to:

  • Work with individuals who have complex needs including co-occurring disorders of intellectual and developmental disabilities, mental health and/or substance use.
  • Exercise sound judgment in appraising situations and making decisions regarding complex situations.
  • Organize workload and manage time effectively.
  • Complete case management core competencies within required timeframe. 
  • Act on personal initiative using sound judgment. 
  • Convey information, ideas, and facts both orally and in writing to supervisors, colleagues, and individuals, inside and outside the county, using language and a format the audience will best understand.   
  • Utilize computers and other technology to communicate in written and verbal forms (e.g., word processing, texts, emails, spreadsheets, department-specific software).
  • Effectively relate well with others, including supervisors, colleagues, and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.  
  • Take personal responsibility for the quality and timeliness of work. Show up to work on time, and follows instructions, policies, and procedures. Meet productivity standards, deadlines, and work schedules.   

WORKING CONDITIONS

Job Conditions: Normal working hours, however, flexibility and additional hours are required to meet the demands of crisis intervention, protective services interventions, and other demands of the job. Position requires exposure to people with a wide variety of behaviors and may involve exposure to potentially dangerous situations.

 

Work Location: Lincoln County 

 

Travel: Job duties require frequent travel to locations outside of the program office, including out of County, to other agencies, programs and to individual’s homes. Close coordination with the Oregon Office for Developmental Disability Services (ODDS) is required which may involve periodic travel out of county for ODDS sponsored meetings, and trainings.

Physical capability to handle high mobility demands and complex personal interactions.

 

  • Physical Ability: Tasks involve the ability to exert very moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (12-20 pounds).
  • Sensory Requirements: Some tasks require visual perception and discrimination. Some tasks require oral communications ability. Some tasks require the ability to perceive and discriminate sounds.
  • Environmental Factors: Tasks are regularly performed without exposure to adverse environmental conditions, such as dirt, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards, animals/wildlife, toxic/poisonous agents, violence, disease, or pathogenic substances.
