QMHP (Lead Case Worker)

MADO Healthcare Douglas Park is looking for a QMHP (Lead Case Worker) head up the Social Services Team!

Summary

The QMHP (Lead Case Worker) works with clients and mental health professionals to independently make a clinical assessment, certify a diagnosis and recommend treatment for persons with a mental illness.

• Ensures compliance with Illinois Department Public Health (IDPH), Commission on Accreditation of Rehabilitation Facilities (CARF) and Admissions requirements

• Ensures client orientation is completed for each new admission
• Directs the provision of services using psychological rehabilitation and emphasizing wellness, resilience and recovery
• Provides crisis de-escalation and intervention
• Assists in resolving client and family grievances in a timely manner
• Ensures the facilitation of all aspects of a client’s plan of care including assessments and treatment planning, communication with managed care organizations (MCOs) and community-based providers
• Works closely with the interdisciplinary team to provide feedback and maintain communication
• Hires, trains, schedules, directs, evaluates and provides oversight to the social services department, staff, including case managers, case manager assistants and group leaders

Requirements

Master's Degree in a Social Services field

Illinois licensed clinical social worker (LCSW) or licensed clinical professional counselor (LCPC). Two to five years’ clinical experience. Supervisory experience preferred.
