Responsibilities:
- Team Leadership: Lead and manage the intake team, providing guidance and support.
- Care Plan Management
- Quality Assurance: Monitor and evaluate the quality of intake services, identifying areas for improvement and implementing corrective actions as needed.
- Collaboration: Work closely with internal departments, including care coordination, scheduling, and billing, to facilitate smooth transitions and continuity of care.Qualifications:
- Minimum of 1 year of experience in a leadership role, preferably in the home care or healthcare industry.
- Strong understanding of the home care intake process, including client assessment and care planning preferred
- Excellent communication and interpersonal skills, with the ability to interact effectively with clients and team members.
- Ability to work collaboratively in a team environment and adapt to changing priorities and deadlines.
