JOB DESCRIPTION:
- Perform physical exams and patient observations.
- Record patients' medical histories and symptoms.
- Create patient care plans and contribute to existing ones.
- Order, administer, and analyze diagnostic tests.
- Monitor and operate medical equipment.
- Diagnose health issues and create a plan of care for patients.
- Administer medicine and other treatments.
- Detect changes in patients' health and modify treatment plans, as needed.
- Consult with healthcare professionals and families.
- Train patients and their families to manage and prevent illness and injury.
- Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
- Facilitate the patient's transition within and between health care settings.
- Monitor and provider start-up status of clinics.
- Provide coverage to additional clinic locations, as needed and assigned.
- 2 years of experience in family medicine as a Nurse Practitioner/Physician Assistant.
- Graduate of an accredited school of Nursing/Physician Assistant Program.
- Current Registered PALicensure in State of Texas.
- Current PA Licensure in State of Texas.
- Current Board Certification in adult primary care or family practice specialty area.
- Current Medicare provider enrollment with active PTAN number.
- Current TX Medicaid provider enrollment with active TPI number.
- Current provider card in Basic Life Support (BLS).
- Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hire.
- Current DEA License, or ability to obtain DEA license upon hire.
- Experience in geriatric care preferred.
- Previous experience with Athena EMR, preferred.
