Summary:
Under the supervision of the Primary Care Clinic Manager/Medical Director and within the RN's scope of practice, provides a variety of medical procedures and coordinates comprehensive healthcare. Performs nursing visits, support duties, and patient follow-up, including care coordination and health education. Supports primary care department functions as needed.

- This position requires compliance with MSHS’ compliance standards, including the Code of Conduct, Compliance Program, and policies and procedures. Such compliance will be considered part of the regular performance evaluation.
- Responsible for delivering patient care through the nursing assessment, planning, implementation, and evaluation process.
- Implements physician’s orders, administers medications, starts IVs, performs treatments, procedures, and special tests, and documents treatment as company policy requires.
- Collaborates with providers and stakeholders to apply sound nursing judgment in patient care coordination.
- Contacts patients for follow-up care, if needed, following a hospital admission or emergency department visit.
- Collaborates with the Primary Care Clinic Manager and EHR Trainer to ensure competency-based training and privileging of staff.
- Manages the VacTrAK vaccine system. Works with assigned staff on the following functions: monitoring temperatures, following inventory, ordering vaccines, filing required reports, educating staff, and following backup procedures in case of power outages.
- Assists with maintaining clinic equipment, including keeping an adequate inventory of clinic supplies, and coordinates acquiring required medical supplies necessary to function efficiently.
- Patient education and chronic care management services, including care coordination.
- Monitors providers’ PAQs and tasks in their absence:
- Refills medications per standing orders; and
- Notifies other providers of abnormal or critical lab values that require immediate attention.
- Supports primary care department functions within the RN scope of practice.
- Maintains accurate, detailed reports and records.
- Assists in policy and procedure development, review and implementation.
- Serves as a resource for non-medical staff regarding medical protocols and emergencies.
Nonessential Duties
- Assist team members as needed to provide excellent patient care.
- Cross-train for other positions within FQHC.
- Other related duties as assigned.
Working Conditions
- Work is performed primarily inside a heated, ventilated, and well-lighted environment.
- Job is considered low-risk for hazard exposure.
- MSHS is a nonsmoking campus.

- Specific vision abilities this job requires include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Minimum Education and Experience:
- Current professional license (RN; Associate, BS degree in Nursing);
- Two years of professional nursing experience. Prior outpatient primary care and/or psychiatric experience is preferred.
- Have general knowledge of EMR/EHR software and Microsoft Office programs
- Ability to read and interpret documents such as safety rules, operations and maintenance instructions, and procedure manuals
General Requirements:
- Clear background check (subject to DHSS background check)
- Current driver’s license; clean driving record
- Current tuberculosis screening test result
- First Aid and BLS certifications must be obtained within two weeks of hire. CPI (Nonviolent Crisis Intervention) certification must be obtained when offered through the agency; may substitute the current MANDT certificate for CPI
