Critical Care in a Post-Acute Setting.
Remember the days when elective surgeries, acute medical episodes, chronic pain all treated through a hospital stay? Today, they sometimes are, but not without complications or comorbidities.
Understanding the post-acute care needs of our seniors is our specialty. Our Medical Director and APRN support us with daily presence on the floors. We invest in equipment and people to assure proper diagnosis and treatment of those age-related diagnosis. We have uncommon staffing patterns for a skilled nursing facility. Creating this position is a perfect example. We are hiring an Admissions Nurse to support the subacute unit with new admissions. Very likely, you will be the first point of contact for new admissions. You will set the stage for the rehab stay.
Your experience allows for critical thinking skills necessary to establish care delivery. Understanding diagnostics is the foundation for the plan of care. We will trust and rely on your clinical acumen to drive both.
Our facility is 3 miles from the hospital. The only difference between their last day in the hospital and the first day with us, is the patient’s ride in the ambulance. The care needs do not change. Your advanced clinical skills will successfully bridge the gap between locations. By your direction, nurses are prepared to treat, and the patient/families eagerly participate in their recovery.
We thrive ourselves on employing nurses who want to use their specialty skills. Supporting you with a dynamic nurse leadership team will foster team learning and advancement. You will have every opportunity to “pour your wisdom” into the hearts and minds of our rising stars.
We lean on technology to communicate care. Understanding and personal comfort with EHR systems is critical. You understand the regulatory nature of post-acute care. You will be a source of strength for our staff who are intimidated by the survey process. You will also build confidence in physician communications. Teacher, protector, counselor: you mark the success of their journey.
Nursing careers provide endless options. Perhaps you have interest in Assisted Living, Memory Care or Alzheimer’s Care. Our blend of nursing services offers you an array of opportunity, regardless of your heartfelt passion today. This tenured nursing staff allows our seniors to adopt us not only care professionals, but family.
Unique to this position is flexible hours to accommodate unpredicted admissions. Unique to this position is to coordinate care effectively and efficiently so that you are freed up to do what you love most: nursing care.
Trying anything new requires a forward-thinking attitude. Blaze the trail toward something new.
We are looking for the perfect you.
