Essential Job Functions
· Minimum of one year experience in home healthcare and infusion desired
· Excellent organizational and interpersonal skills.
· Leadership and initiative in processing referrals.
· Basic knowledge or familiarity in medical terminology and diagnostic codes.
· Basic knowledge or familiarity with insurance verification, healthcare benefits assessment and paperwork.
Duties and Requirements
· Coordinate patient care with ancillary personnel
· Present a positive professional image and promote the company in all work situations
· Responsible for developing a good working relationship with patients, families, physicians, nurses and other referral sources.
· Correlates referral information on new patients in order to streamline communication of data related to the plan of care, administration of home therapies.
· Obtains authorization for services as needed for payors. Gather pertinent clinical information in and disseminate to clinical staff, enter all information necessary in the computer to produce a complete patient record.
· Obtains weekly clinical reports and obtains authorization extensions as necessary.
· Participates in the coordination of deliveries to the patient’s home to ensure they are cared for in an efficient and convenient manner.
· The referral of patients to a home health agency, etc. as necessary
· Complete all paperwork as necessary for the reimbursement process.
· Participate in departmental PI/QI activities as necessary.
Job Type: Full-time
Pay: From $18.00 per hour
