We are hiring for the following:
Position: Medical Records Clerk
Schedule: Days
Location: Arlington Detention Center
Pay Rate: $20/hr
Responsibilities:
- Create and maintain new medical records and existing medical records by gathering appropriate record folders and contents
- Gather patient information by collecting demographic information from a variety of sources; interact with institutional records office and sending facilities
- Initiate health records for inmates by creating and processing the patient care record folder
- Maintain confidentiality of all medical record information; maintain health records in accordance to established policies and procedures
- Monitor and maintain accountability for all health records on file
- Maintain record availability by processing charts into the department and adhere to chart mark-off procedures; facilitate chart location activities
- Pull and prepare health records for Practitioner to include: Medical, Dental and Mental Health, Sick Call, Chronic Care, and Transfers
- Maintain quality results by following MEDIKO, state and federal, facility, HIPAA, ACA and NCCHC standards
- Maintain continuity of work operations by documenting and communicating actions, irregularities, and continuing needs
- Maintain all active health records in alphabetical order
- Understand and use applicable computer system to update and retrieve inmate patient data
- Forward all diagnostic test results to the appropriate Practitioner for review and countersignature
- Comply with the rules, directives, policies, procedures and protocols of MEDIKO and the facility
- Order and maintain appropriate levels of health services forms and clerical supplies
- Review records for completeness
- Flag records, x-rays, MAR’s and lab test results in medical records, as indicated or requested by medical staff
- Meet with the HSA on a regular basis to review medical records
- Follow process for Release of Information (ROI) from outside providers, law enforcement or correctional staff, attorneys, doctors, hospitals and other health facilities
- Copy all medical records for previous inmate patients as designated by court/attorney order or as indicated on a properly executed medical release form
- Serve and protect the facility by adhering to professional standards, policies and procedures
- Prepare copies of medical records for transfers
- Participate in Quality Improvement activities
- Participate in cost containment goals, standards, and activities
- Maintain working knowledge of facility safety regulations and procedures at all times
- Communicate clearly, effectively and constructively with all MEDIKO staff and all correctional facility contacts (including detention staff)
- Scan in medical information into the Electronic Medical Record (EMR), as applicable
- Assist other team members with their work and work overtime as required
- Perform other duties as assigned
Qualifications
- Must be eighteen (18) years of age or older
- High School graduate with at least one (1) year of similar training and/or experience
- Associate’s degree or Bachelor’s degree in Health Information preferred
- Minimum of one (1) year of Records Management experience
- Active CPR/BLS certificate required (American Heart Association/American Red Cross guideline approved)
- Experience in medical data entry, and accurately type at least 50 WPM
- Familiar with the problem-oriented Medical Record System (SOAPE) essential
